Nvidia Quadro 4000 CUDA issues OSX 10.7.5

Hi everybody!

Have a Mac Pro tower with an Nvid Quadra 4000 installed.

Having issues with CUDA and trying to determine the nature of the problem. 

Here is what i'm working with:

After running the GPU sniffer for Premiere, it appears it's only picking up on the OpenCL device:

I've looked into the following fix:

But to no avail.

Anyone have any answers?
